Penile carcinoma, also called penile cancer, is a rare malignant tumor of the penis. It occurs less often in countries with high hygiene standards. In many cases, penile cancer is a squamous cell carcinoma. This means it is a tumor that starts in the surface layer of the skin or mucous membrane. Penile carcinoma often develops on the glans, foreskin, or coronal sulcus. Today, doctors mainly distinguish between HPV related and non HPV related forms. Early detection is very important. In early stages, the chances of recovery are much better. Organ preserving treatment is also often possible.

Penile Cancer Causes and Risk Factors
Penile cancer usually does not develop from one single cause. In many cases, several risk factors act together. Long term damage or changes to the surface skin or mucous membrane of the penis often play a key role. These changes can lead to penile carcinoma over time. About half of tumors are linked to HPV. Other tumors develop independently of HPV. They are often linked to chronic inflammation, local irritation, or previous tissue damage. This distinction is important. It can influence how the tumor develops and behaves.
The main risk factors include:
- HPV infection, especially with high risk types 16 and 18
- Chronic changes in the skin or mucous membrane, not related to HPV
- Chronic inflammation, especially balanoposthitis
- Lichen sclerosus
- Phimosis
- Poor intimate hygiene
- Smoking
The most important warning signs of penile carcinoma are new or long lasting changes on the glans, foreskin, or penile skin. These changes may include redness that does not heal, hardening, a sore area, bleeding, bad smelling discharge, or palpable lumps. A urologist should check these symptoms early. The chances of recovery are much better in early stages.
Good intimate hygiene can help with prevention. Treating phimosis, stopping smoking, and avoiding chronic inflammation can also reduce risk. Circumcision may also have a protective effect. This is especially true when it helps prevent recurring irritation or hygiene problems.
HPV plays a particularly important role. This is especially true for the high risk types 16 and 18. These types are involved in a relevant share of penile carcinomas. HPV vaccination is therefore considered an important part of prevention. Condom use can also help reduce HPV transmission. Both may help prevent HPV related precancerous changes and probably some penile carcinomas.
Penile Cancer Symptoms
Penile cancer symptoms often develop slowly. In early stages, changes are often underestimated. They may look harmless at first. Typical early signs include flat or slightly raised redness, small hardened areas, or a wound that does not heal fully. These changes are often painless. Because of this, they can be overlooked or mistaken for mild inflammation. As the disease progresses, a growing tumor may develop.
Typical symptoms and warning signs of penile cancer include:
- Flat, hardened, or raised redness
- A wound or mucosal change that does not heal
- Ulceration
- A lump or outward growing tumor
- Coatings, discharge, or bad smell
- Pain or burning, especially in more advanced stages
- Palpable lymph nodes in the groin
In more advanced cases, the tumor grows deeper into the tissue. It can also affect the erectile tissue and the urethra. This may cause pain, bleeding, problems passing urine, or even urinary retention.
The inguinal lymph nodes are also important for prognosis. If they are affected, they may feel hard and swollen in the groin. They are often painless. This finding is clinically important. It has a strong influence on stage, treatment, and prognosis. In short, any new, persistent, or changing lesion on the penis should be checked early by a urologist.
Penile Cancer Diagnosis
The diagnosis of penile carcinoma often starts with a clinical examination. The doctor carefully examines visible or palpable changes on the penis. At the same time, both groin areas are checked. This helps identify enlarged or hardened lymph nodes. The clinical examination can already give clues about stage and prognosis.
A tissue sample is essential to confirm the diagnosis. The tissue is then examined under the microscope. Only this can show whether it is truly penile carcinoma. It also shows how deeply the tumor has grown into the tissue. In most cases, it is a squamous cell carcinoma. This histological classification is the basis for further treatment planning.
Imaging also plays an important role. This is especially true when the local spread needs to be assessed more precisely. MRI of the penis can help show whether deeper structures are affected. These may include the erectile tissue or the urethra. Depending on the findings, CT may also be used. It can help assess lymph nodes and possible distant metastases. If the lymph nodes appear normal during the clinical exam, a lymph node biopsy may be considered. This can detect microscopic lymph node involvement early.
As with many cancers, diagnosis of penile carcinoma combines several steps. These include clinical examination, biopsy, and imaging.
Penile Cancer Treatment and Prognosis
Penile cancer treatment aims to stop tumor growth and achieve recovery. Whenever possible, it also aims to preserve the anatomy and function of the penis. The type of treatment depends on how far the tumor has advanced. It also depends on whether groin lymph nodes are affected and whether distant metastases are present. In early stages, penile carcinoma is often still localized. Early diagnosis is therefore very important. It improves the chances of recovery and can allow organ preserving treatment.
In early or localized stages, doctors usually aim for organ preserving treatment. Depending on the findings, this may include local excision, laser therapy, glans preserving surgery, glansectomy, or in some cases radiation therapy. The goal is to remove the tumor safely. At the same time, treatment should preserve urinary function, sensitivity, and sexual function as far as possible. If the tumor has already grown deeper into the tissue, partial or total penectomy may be necessary. The key point is that the tumor must be removed completely.
Careful assessment and, if needed, treatment of the groin lymph nodes is also important. Lymph node status is one of the most important prognostic factors in penile carcinoma. If the lymph nodes are clinically normal, a lymph node biopsy or detailed staging may be used, depending on the risk profile. If lymph node involvement is confirmed, surgery to remove lymph nodes may be needed. Depending on the extent, systemic therapies or multimodal treatment concepts may also be considered. In locally advanced or metastatic stages, chemotherapy is often used. In selected cases, chemoradiotherapy and palliative measures may also be used. The goal is to control tumor growth and relieve symptoms.
Complementary medicine can support cancer treatment. It may help reduce side effects and support quality of life during therapy. For example, acupuncture may help relieve pain, fatigue, sleep disorders, anxiety, and stress. Some phytotherapeutic preparations or aromatherapy may also be used in a symptom oriented way. They may support digestion, sleep, or emotional strain.
In integrative oncology, hyperthermia is also used as a supportive component. Most penile cancers are squamous cell carcinomas. This allows doctors to draw on stronger evidence from other body regions. In squamous cell carcinomas, clinical evidence suggests that local heat may make tumor cells more sensitive to radiation therapy or chemotherapy. However, large specific studies on rare penile carcinoma are still lacking. This means the principle can only be transferred with care. It may help support conventional treatment as well as possible.
These approaches should never replace standard oncological treatment. They should only be used as supportive measures. Their goal is to improve subjective well being and quality of life as much as possible.
