Esophageal cancer, also called esophageal carcinoma, is a malignant tumor of the esophagus. The esophagus is a long muscular tube. It connects the mouth and throat with the stomach. The tumor often starts in the inner lining of the esophagus. Doctors mainly distinguish between two main types of esophageal cancer. One is squamous cell carcinoma. It develops from the surface cells of the lining, usually in the upper or middle part of the esophagus. The other is adenocarcinoma. It is more common in the lower part of the esophagus, near the entrance to the stomach. It often develops after long-term changes in the lining, such as Barrett’s esophagus. These two forms have different risk factors and develop in different ways. In both cases, abnormal cells grow in an uncontrolled way. Over time, this can affect how the esophagus works. The cancer cells can also grow into deeper tissue layers and damage nearby structures.
Esophageal cancer usually develops over many years. Repeated irritation and inflammation can change the cells of the esophageal lining. These changes can disturb normal growth and repair processes. This may increase the risk of esophageal carcinoma. Compared with many other cancers, esophageal cancer is less common. However, lymph node metastases are often already present at the time of diagnosis. In Western countries, adenocarcinoma of the esophagus has become more common. This is mainly linked to lifestyle factors such as obesity, lack of exercise, frequent reflux, and diet.

Esophageal Cancer Causes and Risk Factors
Esophageal cancer usually develops through a process that lasts for many years. During this time, the lining of the esophagus is repeatedly irritated, damaged, and repaired. This can lead to genetic changes in the cells of the lining. These changes may disrupt normal growth and repair control. The causes of esophageal cancer are therefore closely linked to certain risk factors and previous diseases. The two main types of esophageal cancer also differ in how they develop and in their risk factors.
In squamous cell carcinoma, the cancer cells develop from the surface cells of the esophageal lining. This usually happens in the upper or middle part of the esophagus. Long-term irritation of the lining plays an important role. Important risk factors include:
- Tobacco use of any kind
- Alcohol consumption
- The combination of smoking and alcohol
- Poor oral and dental hygiene and chronic inflammation in the mouth and throat
- Chronic damage, such as achalasia or strictures after chemical injury to the esophagus
Adenocarcinoma of the esophagus usually develops in the lower part of the esophagus. This is close to the transition to the stomach. Chronic reflux and the resulting changes in the lining are especially important. The main risk factors include:
- Chronic reflux disease
- Barrett’s esophagus, a known precancerous condition
- obesity
- A diet high in fat and low in fiber
- Lack of exercise
- A family history of Barrett’s esophagus or adenocarcinoma of the esophagus
In most cases, no single clear cause can be found for either type of esophageal cancer. It is usually a combination of long-term environmental and lifestyle factors and individual susceptibility. Alcohol and tobacco are especially important in squamous cell carcinoma. Reflux, Barrett’s esophagus, and excess body weight are more important in adenocarcinoma. Genetic or hereditary factors may also increase the sensitivity of the lining. They may also affect how well cells repair damage.
Esophageal Cancer Symptoms
Esophageal cancer often develops slowly over a long period of time. Early symptoms can therefore be vague and easy to misunderstand. At first, people may notice mild pressure or a foreign body feeling behind the breastbone. These symptoms often occur when eating solid food. Later, they may also occur when drinking liquids. The esophagus can stretch quite well. For this reason, a tumor is often only noticed once it has already caused a more significant narrowing.
Typical esophageal cancer symptoms may include:
- Difficulty swallowing, also called dysphagia
- Painful swallowing, also called odynophagia
- Pressure or tightness behind the breastbone
- Loss of appetite and feeling full quickly
- Recurrent heartburn or acid reflux
- Nausea or vomiting
- Hoarseness or chronic cough
- Blood in the stool, often appearing as black stool
Important warning signs should be taken seriously. These include new or slowly worsening swallowing difficulties. Another warning sign is the feeling that food often gets stuck in the throat or behind the breastbone. Unexplained and clear weight loss over weeks or months should also be checked. Frequent vomiting or blood in vomit, especially together with heartburn and swallowing problems, should be assessed by a doctor promptly.
Esophageal Cancer Diagnosis
The diagnosis of esophageal cancer often starts with rather unspecific symptoms. These may include swallowing problems, pressure behind the breastbone, or unintentional weight loss. The first step is always a detailed medical history. The doctor asks about symptoms, how long they have been present, and possible risk factors. These include smoking, alcohol, reflux, and previous diseases. A physical examination follows. However, the diagnosis is confirmed by an examination of the esophagus and stomach. This is called an esophago-gastro-duodenoscopy. During this examination, a thin flexible tube with a camera is passed through the mouth into the esophagus. The doctor can see abnormal changes in the lining and take a biopsy. Every suspicious change is checked with a biopsy. Small tissue samples are taken and then examined under a microscope. Only the tissue result can confirm esophageal carcinoma.
The next step is to determine how far the cancer has spread. This is called staging. Further imaging tests are used for this. Endoscopic ultrasound diagnostics can show how deeply the tumor has grown into the wall. It can also help assess nearby lymph nodes. CT scans of the chest and abdomen are often added. They help detect metastases in the body. In some cases, further tests may be needed. This is especially true for tumors near the transition to the stomach. These tests may include a stomach endoscopy, bronchoscopy, or diagnostic laparoscopy. They can help detect even small tumor deposits that may not be visible on CT scans.
Blood tests are also part of the diagnostic workup for esophageal carcinoma. They usually include a blood count and inflammation markers. It is important to know that there is no specific tumor marker that can reliably detect esophageal cancer. Markers such as CEA, CA 19-9, or SCC may be elevated in some cases. However, they are not specific. They are not suitable for early detection or for confirming the diagnosis. In individual cases, tumor markers may help with follow-up. This is only useful if the values were already elevated before treatment.
Esophageal Cancer Treatment and Prognosis
Esophageal cancer treatment is often complex. It usually requires cooperation between several medical specialties. The aim is to remove or destroy the tumor completely and achieve a cure when possible. At the same time, doctors try to preserve or restore the function of the esophagus. The exact treatment plan is adapted to each person. It depends on the tumor stage, the exact tumor location, the person’s general health, and possible other diseases.
If the tumor is still at an early stage, endoscopic removal may be enough. This applies when the tumor is limited to the surface layers of the lining. It also means that lymph nodes and nearby tissue are not affected. In these early stages, the chances of cure can be good. During endoscopic treatment, the affected area is removed. This option is mainly possible when the tumor is small and clearly limited. Close follow-up is still recommended after removal. The lining remains at increased risk.
Once the tumor has grown deeper into the wall of the esophagus, surgery becomes more important. The same applies if nearby lymph nodes are affected. The operation is called an esophagectomy. During this procedure, the affected section of the esophagus is removed. Nearby lymph nodes are usually removed as well. To allow food to pass again, the esophagus is often reconstructed. This is usually done with part of the stomach. Less often, a piece of bowel is used. Surgery is often combined with chemotherapy or radiation therapy before or after the operation. Neoadjuvant treatment aims to shrink the tumor. This can improve the chance of complete removal.
In squamous cell carcinoma, definitive chemoradiotherapy without surgery may be an option. This is especially relevant when surgery is not possible or not reasonable for medical reasons. In advanced stages, when cure is no longer possible, chemotherapy is often used. The goal is to slow tumor growth and maintain quality of life for as long as possible. In selected cases, immunotherapy may also be used. This can apply, for example, to adenocarcinomas with certain markers.
Supportive care and nutritional support are central parts of treatment. Many people already have swallowing problems and weight loss before treatment begins. Early nutritional care is therefore important. If cure is no longer possible, or if the esophagus is severely narrowed, a stent may be placed. This can make swallowing easier again.
Complementary medicine, such as acupuncture, may help reduce pain, fatigue, sleep disorders, anxiety, and stress. It may also help support quality of life during treatment. In some cases, oncological hyperthermia may also be offered as a supportive measure. More specifically, local hyperthermia is usually used together with chemotherapy or radiation therapy. Its aim is to relieve symptoms and support local treatment. Selected phytotherapeutic preparations may also be used in a symptom-oriented way. They may support digestion, sleep, or the nervous system. None of these approaches replace standard oncological treatment. They are used as complementary measures to support quality of life and subjective well-being.
