It can also affect the bone marrow, lymph nodes, and often the spleen and liver. In CLL, abnormal B lymphocytes multiply in an uncontrolled way. Over time, they build up in the body. This can interfere with normal blood formation. A typical feature of chronic lymphocytic leukemia is that it often develops slowly. It is therefore often found by chance during a blood test. Even though CLL usually progresses slowly, it can lead to swollen lymph nodes, increased susceptibility to infections, anemia, and exhaustion.

Chronic Lymphocytic Leukemia (CLL): Causes and Risk Factors

The causes of CLL are not clear in most cases. This type of blood cancer usually cannot be traced back to one single cause. The disease is based on abnormal changes in B lymphocytes. These immune cells slowly multiply in an uncontrolled way. CLL is therefore usually linked to several changes that develop and build up over time.

The most important known risk factors include:

  • Older age
  • Genetic predisposition or a positive family history
  • Environmental factors, such as organic solvents or radiation

Chronic lymphocytic leukemia also occurs at different rates in different population groups. It is seen much less often in people of East Asian, South Asian, or Indigenous American ancestry than in people of European ancestry. This may suggest that genetic factors play a role in CLL. Men are also affected more often than women.

Besides age and genetic predisposition, external factors are also being studied. These include ionizing radiation and certain chemical exposures, such as organic solvents.

Many people with these risk factors never develop CLL. At the same time, some people with CLL have no clearly identifiable trigger. It is important to detect changes in the blood count early. This helps doctors clarify whether CLL or another blood cancer may be present.

Chronic lymphocytic leukemia often causes no clear symptoms for a long time. It is therefore often found by chance during a blood test. When symptoms occur, they usually develop because abnormal B lymphocytes build up in the blood, bone marrow, lymph nodes, and organs. They can also occur because the immune system becomes weaker.

At first, many people notice general symptoms, such as:

  • Fatigue
  • Weakness
  • Unintentional weight loss
  • Night sweats
  • Slightly raised temperature or fever

Because CLL usually develops slowly, these first signs are often not linked to a blood disease. Later, enlarged lymph nodes may become noticeable. They are usually painless and may remain enlarged for a long time. The spleen or liver may also become enlarged because lymphocytes can collect there.

A key issue in CLL is that the immune system often becomes weaker over time. The disease often causes a defect in B cells, which can lead to antibody deficiency. This may increase susceptibility to infections.

Important signs include:

  • Frequent bacterial infections
  • Infections with a severe course
  • Fungal infections, such as thrush
  • Viral infections, especially shingles

Other symptoms can also occur. They may be linked to the spread of the disease or to immune-related changes.

These include, for example:

  • Swollen lymph nodes
  • Enlarged spleen or liver
  • Chronic itching
  • Chronic urticaria

Typical warning signs include persistent fatigue, recurrent infections, painless swollen lymph nodes, and unintentional weight loss. A combination of several symptoms should also be checked by a doctor. The same applies if general health clearly worsens. Although CLL usually progresses slowly, early and accurate assessment is important. It helps doctors choose the right treatment and avoid possible complications.

Chronic Lymphocytic Leukemia (CLL): Diagnosis

Chronic lymphocytic leukemia is often found by chance during routine blood tests. It is less often discovered because of typical symptoms. A higher number of lymphocytes in the blood can raise suspicion of CLL. Because CLL may remain clinically silent for a long time, correct interpretation of blood values is especially important.

An important step in diagnosis is the differential blood count. It can often show lymphocytosis, which means an increased number of lymphatic cells in the blood. A blood smear may also be examined under a microscope. However, this is not enough to confirm the diagnosis. The key test is immunophenotyping by flow cytometry using a blood sample. This can detect the characteristic abnormal B lymphocytes. The test is very important because it helps distinguish CLL from other lymphatic diseases.

Molecular genetic and cytogenetic tests may also be relevant. They can detect genetic changes and mutations. This can help doctors plan the most suitable treatment.

A bone marrow puncture is not always needed to diagnose CLL. In some situations, it may be useful. This can be the case before treatment starts or if it is unclear how strongly blood formation is already affected. Imaging tests such as ultrasound diagnostics or CT can also help. They can assess enlarged lymph nodes, possible organ involvement, or disease progression.

Chronic Lymphocytic Leukemia (CLL): Treatment and Prognosis

Treatment for chronic lymphocytic leukemia is mainly based on disease activity, general health, risk profile, and other medical conditions. One important point is that CLL does not always need immediate treatment. If there are no symptoms, the blood count is not significantly worse, and the disease is not progressing, doctors often monitor CLL closely at first. This approach is called “watch and wait.”

If CLL does require treatment, targeted therapies are often used today. These treatments have changed CLL therapy significantly in recent years. They are often more effective and easier to manage. Chemoimmunotherapy still plays a role in some cases. However, it is now much less central than in the past. High risk features are especially important when choosing treatment. Some of these cases respond less well to classic chemotherapy. They are therefore often treated with targeted approaches.

CLL is usually not curable with standard drug therapies alone. The only potentially curative option is allogeneic stem cell transplantation. It may be considered in selected cases. These include treatment resistant disease, early relapse, or Richter transformation. The best treatment option depends strongly on the risk profile and overall health.

Alongside medical treatment, complementary medicine may help support quality of life and relieve symptoms in some people with CLL. In supportive oncology, approaches such as acupuncture, movement, relaxation techniques, yoga, mindfulness based exercises, and Traditional Chinese Medicine are often used. They may support people with symptoms such as exhaustion, sleep disorders, pain, or inner restlessness. Another physical approach is oncological hyperthermia. In this context, mild whole-body hyperthermia is used in particular. Because CLL is a systemic disease, this controlled warming of the body aims to gently stimulate the immune response and support general vitality. Targeted nutritional changes or nutritional supplements, such as vitamin D, may also be useful. They can help correct deficiencies early, prevent weight loss, and support the body during a stressful phase.

The prognosis of CLL varies widely. It cannot be judged by the diagnosis alone. The course of the disease mainly depends on age, clinical stage, other medical conditions, and genetic changes such as TP53 mutations. Overall, life expectancy has improved significantly because of targeted therapies. A definitive cure is usually not possible. However, long term disease control with good quality of life can often be achieved.
